Perhaps you have been inspired to add your story to the site. To begin the process, fill out the form below. The only items on the form that are required, are your name (it will
not be used on the site unless you put it in the text of your bio), a valid e-mail, and your chosen site name (max.of 8 characters). This site name will be used to identify your
bio.
Please fill in all of the other information that you can. This information (but not your name and address) will appear at the top of your bio. They will serve as a introduction to your story, helping visitors decide which bios they are interested in reading. If your bio is ready and short, you may copy it into the bio space. That box is bigger than it looks. Feel free to fill it with about a page of material. If you have not prepared your bio yet, or if it is longer than a page, please e-mail it to me at webmaster@guaifenesin.net when it is ready. Include the site name you have chosen with all correspondence about your bio. If you feel that you would like me to edit your work, just let me know in the comments section of the form or with the bio itself. Don't worry about your writing not being good enough for the web. Just let me know you would like help, and I'll do what I can.
A few suggestions about the material in your biography..
First make this a real bio, not just a report of current events. Use the guai-support list to report your current progress or to seek advice and support. This is the time to collect your thoughts, and to record your unique overall experience with this DD and it's treatment. You've probably been waiting to do this; to tell your story to all who want to listen.
Well this is your chance. What have you learned? What is of value that you can share with others?
You can cover your life's history with this DD, or focus on just one or a few aspects of your unique story. Perhaps there is one symptom which has characterized your illness, or maybe you want to discuss the effect of this illness on your family life or career. This is your forum. Make it any length you want. You will be able to add to your bio later as you see fit; don't think of it as a static document.
Don't be shy. Your own experience can't be told by someone else; only you can do this.
How about including a picture? You can include a small one with your bio as an attachment or in the document. If you would like help with the writing, let me know. The first step is to fill out a submission form. Ready? Set? Go!

Your Real Name - This is required for administration purposes, but will not be placed on the site unless you include it in your bio information
Site Name - Up to 8 alphanumeric characters. You make this up yourself. It will be used as the name of your bio. Use this name for all correspondence about your bio.
E-Mail - An e-mail where we can contact you. This e-mail will not be included in the site unless you specifically allow this.
Major Symptoms - List up to three of your major symptoms. This is to help the reader pick out which bios match their interest.
Descriptive Words/Phrases - List up to 4 words or phrases which best describe you, or your overall bio, for example, "lower back pain, male, family history,
headaches". These descriptions would be for the bio of a man who is using his bio to describe how the DD has manifested in several generations of his family.
His major symptoms might be back pain and headaches.
Put Your E-Mail on Site? - Answer yes, and include your e-mail if you want the readers to have an e-mail address to contact you directly.
Other Helpful Things/Useless Things - List here other things which you have tried for your DD which either were most helpful or which were not helpful at all.
these will help the reader to select other things besides guai which may be useful to them
Comments - Any additional information or comments which you would like the webmaster to know.
Cut and Paste in Your Bio - Include your bio here if it is ready and short.
Otherwise send an e-mail to the webmaster@guaifenesin.net attaching the bio
when it is ready. Be sure to include the site name you have chosen for your bio in that e-mail. You can submit a bio in the following formats: HTML, Microsoft
Word, Microsoft rtf (produced by wordpad), Text (txt) produced by notepad or another text editor. When writing the bio, don't enter carriage returns into the
text after every line. Separate paragraphs with an empty line. Use other formatting such as centering as desired. Please use a spell checker on your final document if you can.
There are always changes which occur when other document formats are converted to HTML. I will do everything to retain the look of your original
document. If you have your material in a format not mentioned above, send it anyway, I will do what I can to convert it for the web.
Don't think you can write well enough? Just do the best you can, and ask for me to edit your material. Feel free to ask for any other help you need.
